What is Neighbourhood Heath?

The NHS is changing the way health and care services are delivered.

Instead of organisations working separately, local services are coming together as Neighbourhood Health Teams. This means healthcare professionals, community organisations and local residents can work together to better understand what people need and provide more joined-up care closer to home.

Our neighbourhood covers:

  • Poplar

  • Limehouse

  • Isle of Dogs

Together, we're building healthier communities where prevention, wellbeing and community support are just as important as treatment.

Our partners

We work alongside many local partners, including:

  • GP Practices

  • Community Health Services

  • Tower Hamlets Council

  • Local charities and voluntary organisations

  • Social Prescribing teams

  • Mental health services

  • Community groups

  • Residents and volunteers

  • Everyone has a role in improving the health of our neighbourhood.

Current Projects

  • Improving pre-diabetes and type two diabetes among Bangladeshi community

  • Communities Keeping Well - community centred approach to addressing long term conditions

  • Supporting housebound patients with befriending support
