Network Team

  • Chris Ley

    NETWORK DIRECTOR

    Chris has worked as Network Director of the Healthy Island Partnership (HIP) on the Isle of Dogs for the past 1 ½ years but has worked in Tower Hamlets for over 23yrs in various Health and Public Sector organisations.

    As well as leading the HIP team he also runs the neighbouring Poplar & Limehouse Health Network and is the strategic lead for the Social Prescribing Service in Tower Hamlets. Chris is also Vice-Chair of the Tower Hamlets Council for Voluntary Services so has a particular passion for connecting health services into local voluntary groups.

  • Lisa Bone

    NETWORK COORDINATOR

    As the Network coordinator, I am involved with various projects to improve patient care for the four practices on the Isle of dogs. I work closely with the practices ensuring that their NIS performance is managed on an in-depth quarterly basis. I also look into the different care packages ensuring that the requirements are met by each practice.

    Working in network 8 I have realised I work with a great team who put patient care first and support one another.

  • Saif Mehmood

    FINANCE LEAD

    My role is to manage the finances for the network, including the funding for our services and the funding to our practices.

    Accuracy and efficiency should be at the heart of any public health-related finance setup. At the HIP, we use this methodology to ensure our services and practices get the funding they need in a timely manner to serve the community.

  • Dr Mike Fitchett

    CO-CLINICAL WELLBEING TEAM LEAD FOR NETWORK 8.

    I was a GP partner at Island Health from 2000 to 2018 when I retired. I’ve always felt that belonging to a supportive community is so important. I really wanted to help make this happen more on the Island. People’s health benefits hugely from connections, activity and purpose. It’s just great to be involved in the project.

  • Dr Sonal Shah

    GP PARTNER AT ISLAND HEALTH AND HEALTH AND CO-CLINICAL WELLBEING TEAM LEAD FOR NETWORK 8.

    It's been a great experience to co-produce a health and wellbeing project to bridge the gap between primary care and the local community. We hope it helps all those living on the Isle of Dogs.

  • Dr Toby Longwill

    GP PARTNER AND CLINICAL DIRECTOR FOR NETWORK 8

    Toby has worked as a GP at Island Health since 2004 and has been part of the HIP board since the Primary Care Network was created in 2009. During that time he has been involved with several public health events to improve health care on the Island. Toby has been the Clinical Director of the network for the last 2 years and has helped develop the network team which now includes Health Coaches, First Contact Physiotherapists and Clinical Pharmacists.

    HEALTH COACH

    As a health coach, I enable people to see themselves as experts of their own bodies and minds and support them to achieve their health and wellbeing outcomes.

    I specialize in supporting people to understand and manage stress, diabetes, and weight management.

  • Tracey Adebowale-Jones

    HEALTH COACH

    I come from a background of delivering community-based educational services to people who struggle with mental health. I specialise in supporting people to understand and manage their stress and anxiety, sleep and pain and coach people to become empowered to make changes to their self-beliefs that enable a healthier and calmer lifestyle. From a basis of mindfulness, CBT coaching and breathwork I deliver sessions both indoors and outdoors, working alongside the nature that surrounds us.

  • Michael Glover

    FIRST CONTACT PHYSIOTHERAPY PRACTITIONER

    My role is to provide an expert Physiotherapy assessment to patients who suffer from Neuro-Musculoskeletal pathology who may or may not need the input of a GP.

    Patients can expect an expert timely assessment and discussion of their problem and what management is required which may include scans or onward referral.

    Working within Network 8 I enjoy the challenges that our multi-cultural diversity brings on a daily basis

Social Prescribers and Volunteer Coordinator

  • Tracy Tundervary

    SOCIAL PRESCRIBER

    My role as a Social Prescriber is to refer and signpost patients to services in the community that can help them with any non-medical issues, linking patients up to groups and activities can make a huge improvement on their mental & physical health and wellbeing.

    Since the pandemic hit more people are experiencing Social Isolation and Loneliness, working in Network 8 I realise we have such a great team of professionals that care about the wellbeing of the patients, we work closely together to try to ensure every person in need gets the support they require, it is a pleasure to be part of the team.

  • Nickie Tundervary

    VOLUNTEER COORDINATOR

    I would like to engage with patients in our practices in the hope to build a community of volunteers who are willing to give their time to help set up various groups on the island, by running several different group activities that will improve the health and well-being of the local people in the community who will attend these groups.

Pharmacists

  • Matipa Chieza

    JUNIOR PRACTICE-BASED PHARMACIST

    My role entails handling prescription queries, discussing changes to medications with patients and carrying out clinical audits. What I like the most about the network is the collaborative work that takes place across every level.
