Health and wellbeing coaches are one of our health care professionals who help people make changes to their behaviour or lifestyle so that they can live fuller and healthier lives. They are experts in helping individuals to find out what drives them, what’s holding them back and what changes they want to make to better their health and wellbeing.

What Can a Health & Wellbeing Coach Help You With?

This is a non-clinical service that offers people the chance to look at their health condition or challenges differently by making sure that conversations are led by ‘what matters to you.’

Health and wellbeing coaches help people with a range of long term conditions including respiratory, cardiovascular ( including type 2 diabetes and high blood pressure) and stress/ low mood. They can also support people with lifestyle and behaviour changes that help with weight management, diet, physical activity, sleep or other areas that help to improve your health and wellbeing.

What Sessions Do The Health & Wellbeing Coaches Offer?

  • Patients work with an experienced coach face to face in a GP practice, vide or telephone call. These sessions will help you focus on what’s important to you and how you can move forward and they typically last for 45 minutes over 6 sessions

  • A 4 week programme focusing on improving the quality of life of people living with pain for more than 3 months after your body has healed or after trying different ways to treat it. This group is led by first contact physiotherapist and 2 health coaches

  • A 6 week programme to help improve knowledge and confidence in making healthier choices around your sleep, stress, food choices and physical activity.

    E-mail the health coaches for a referral or ask for your GP practice

  • A group that helps to explore issues around worrying, racing thoughts or stress that allows you to learn more about a mindful way to living.

    This session is open to parents at Harbinger Primary School and Cubitt Town Primary School

If you’re registered to one of the GP practices on the Isle of Dogs, you can be referred to a health and wellbeing coach by asking at your GP practice or sending an e-mail to the Social Prescribers thgpcg.socialprescribingNW8@nhs.net
